Please note that updated records need to be sent to the school at the start of each school year. If you have a returning student, please be sure that you are still making this request with your physicians office! Thank you!

Immunization Policy and Physical Record Request 26/27

Public Law, Ch. 154 (introduced as LD798): An Act to Protect Maine Children and Students from Preventable Disease by Repealing Certain Exemptions from the Laws Governing Immunization Requirements was signed by Governor Janet Mills on May 24, 2019. This new law, which takes effect September 1, 2021, requires changes to the following Maine Rules:

05-071 Chapter 126 and 10-144 Chapter 261 - Immunization Requirements for School Children (DOE and Maine CDC Joint Rule

The State of Maine Department of Education (MDOE) requires The Community School (TCS) to have all student immunization records, from a licensed physician on file, along with copies of your child’s last physical results, including updated vision and hearing exams. Please review the MDOE mandated Student Immunization Policy, here.  MDOE and TCS require each child to have had a documented physical within one year or less of the start date of school. 

Please contact your child’s primary care physician to get a copy of their immunization records and last physical sent or emailed to TCS by September 1, 2026.
